We offer comprehensive care packages in addition to single-session options. We are not in network with any insurance providers and do not submit to insurance at this time but can provide you with paperwork to submit on your own after the visit. All visits are via Zoom.

Lynch Syndrome Funding

Project Conquer is Mayberry Memorial’s flagship program that pays for high-risk, unidentified Lynch syndrome Previvors to receive genetic counseling and genetic testing for Lynch syndrome. We offer a special package for genetic counseling and testing for Lynch syndrome that will be completely reimbursed by Project Conquer.

You must apply through Project Conquer and fill out the form below to become a new patient.

You may have Lynch syndrome if you have a family history of colorectal, uterine, stomach, ovarian, pancreatic, urothelial, brain (usually glioblastoma), bile duct, or small intestine cancers, especially if diagnosed before age 50.

Due to state licensure laws, we can only accept patients who will physically be in Alaska, Arizona, California, Colorado, Illinois, Kansas, Maine, Mississippi, Missouri, Nevada, New York, North Carolina, South Carolina, Texas or West Virginia at the time of the visit.
